For homesteaded residential properties, ONLY, this form and borrower contact information must be filed with the RMFM Program, on the web-enabled IT Platform.

Notice of RMFM Program (Exhibit 2) (download copy here) – this form must be included with the Summons served on the defendant in any residential mortgage foreclosure case filed after July 12, 2010. In the 15th Circuit, this form must be printed on light green, light yellow, or light pink paper.

Plaintiff’s Notice of Attending Mediation Through the Use of Communication Equipment (Exhibit 7) (download copy here) – this form gives notice that the Plaintiff’s Representative plans to attend the mediation through the use of communication equipment and designates a person who will be physically present at the mediation with full authority to sign a settlement.

Plaintiff’s Certification Regarding Attendance at Mediation through the Use of Communication Equipment (Exhibit 8) (download copy here) – this form certifies that the Plaintiff’s Representative did attend the mediation through the use of communication equipment. If the mediation results in an impasse, this form must be filed with the court within 5 days after the mediation session.

Certification Regarding Settlement Authority (Exhibit 10) (download copy here) – if the case is not a homesteaded residential property, this form must be filed with the Complaint, identifying the agent of the plaintiff who has full authority to settle.
