Plaintiff Forms
We appreciate your cooperation in this Program by
working with the IT Platform and providing us with
as much contact information for the borrowers as you
can provide.
Liaison: If you plan to file 5 or
more cases, please send the names and contact information
of your liaisons to us, so that we can work with you
to make this transition a smooth one.
IT Platform: Form A and the borrower’s
contact information should be entered electronically
through OUR IT platform. THIS
SHOULD ONLY BE DONE FOR HOMESTEAD CASES.
Bulk Uploading: Our system will
accommodate bulk-uploading of cases (5 or more cases
at a time). The cases will need to be entered into
an Excel spreadsheet in a specified format and the
initial upload will be screened by our software provider.

Fees: $400 administration fee –
payable at the time Form A and contact information

Checks may be made payable to: PBCBA RMFM and mailed
to above address. Please be sure to include
the case name and number on the check.
Refund: $100 for foreclosure counseling,
if the borrower does not proceed to foreclosure counseling.
Instructions for logging
on the web-enabled IT Platform:
Authorization: Each user of this system will need
to be authorized before being given access to the
system. Click here
to access our IT platform. Go to the upper right hand
corner to Login (when you first try to login, it will
be necesary to install Microsoft Silverlight which
it will prompt you to do). When the login screen comes
up, click on “register” and complete the
information. (Note: your user name must be alphanumeric
characters, with no spaces. Your password must be
at least 7 characters long with at least one non-alphanumeric
character.) The completed registration information
will start the authorization process. Once authorized
(which may take up to 24 hours), cases can be entered
into the system and managed.
How to file Form A
and input borrower’s contact information onto
the IT Platform for a new case
General Input information: if there is an OK button
at the bottom of the screen, then click it. Press
the floppy disc icon to save and the green arrow to
go to the next screen. Continue adding information,
saving, and going forward.
Phone numbers should be entered in the xxx-xxx-xxxx
or (xxx) xxx-xxxx formats ONLY.
1. After logging on, the second box on the top left-hand
corner is “Forms”.
2. Click on “Forms” and go to “New
Case Wizard.”
NOTE: Remember to click on the Black floppy disk in
the upper left to save your information each time
you enter data.
3. Fill out the Case Information page of the new case
wizard, you must put a value in the Loan Number field
(even if it is zero)
4. On the Address of Property page make sure to enter
the County as this will be used when printing Forms.
5. On the Link to Existing Plaintiffs page PLEASE
search for the Plaintiff before entering a new one.
You can do this by putting any part of the Plaintiff’s
Company name in the search box and clicking search.
If the proper Plaintiff and Representative are in
the search list then highlight that record and click
Add this Plaintiff. The Plaintiff company will now
be shown on the right as being selected.
If the proper Plaintiff and Representative does not
show up in the search list then click the Green arrow
to go to the next screen. On this screen you can enter
a new record for the Plaintiff and Representative.
Follow the same procedure as in step 5 above to select
or enter the Lawyer for the Plaintiff.
6. On the Defendants Screen of the New Case wizard,
enter the Defendants of the case. Make sure to select
BORROWER from the dropdown field for the Primary Borrower
and CO BORROWER for any CoBorrowers. When entering
a new Defendant, you can copy the address from the
subject property by clicking Copy Address. Clicking
Copy Borrower will copy the Borrowers address to any
Defendant record.
7. Follow the same procedure as in step 5 above to
select or enter the Lawyer for the Defendant. If there
is not one then click the Green arrow to move on without
entering anything.
8. When you reach Confirm New Case wizard, make sure
to Click on Enter Form A Questions. Double click on
the box to the right of the question to select Yes
or No for each question.
9. When you are done you can print Form A which can
be filed with the Court. Be patient and let the form
be generated and for the printer selection screen
to appear.
If you stop in the middle of the wizard, you can
start up the wizard for your case again by doing the
following:
1. Click on Database and then go to Cases
2. Search for respective case and highlight it.
3. Click on the Case details icon at the top of the
screen
4. Now you can click on the Magic Wand at the top
right to start up the case wizard for this case.
5. Any information you have already entered should
be should be displayed as you move through the wizard
and you can pick up where you left off.
Uploading Documents into a Case file: If
you would like to upload an attachment for Form A
(for a long list of Plaintiff representatives, for
example) or upload other documents, such as Exhibits
7 or 8, or Plaintiff’s responses to requests
for documents, return to the case through the case
Database. Click on Database, select Cases, and then
search for the particular case. Once you open the
case details, in the bottom right hand corner, there
is a blue box that states “Select files to upload”.
Click on the “browse” button and then
locate the document to be uploaded and upload it.
How to search for a case in the database:
Once you are in the case database, you can search
for the case by case number or primary defendant name.
Next to both of these fields is a small funnel, click
on the funnel and a search box appears. Go to the
middle and select “contains” as the method
of searching, then type in the name or case number.
By using “contains,” you do not need to
type in the entire name or case number.
